Paediatric Dentistry

First Dental Visit

Protect your child by starting dental visits early. The American Dental Association and American Association of Paediatric Dentistry say, the first dental visit should occur as soon as the first tooth erupts but no later than first birthday of the child. Its best to meet your Paediatric Dentist before any dental problem commences, don't wait until any dental emergencies.

Professional intervention within 6 months after the eruption of the first primary tooth or no later than 12 months of age directed at factors affecting the oral cavity, counselling on oral disease risks and delivery of anticipatory guidance - American Academy of Paediatric Dentistry

•   Early intervention is aimed at preventing or mitigating common paediatric oral diseases and conditions

•   Primary prevention of dental disease is based on timely family education, instruction and motivation for behavioural changes, appropriate fluoride management, early identification of risks and tailored preventive programs.

•   Foundation upon which prevention of oral injuries, management of oral habits, assessments of oral development.

The assessment of risk that a child may have to develop caries at an early age, makes it possible to plan further care. An intense history regarding diet, brushing habits, fluoride use etc. are the first clues. Important factors in the determination of caries are the individual properties of the saliva; the surface structure of the teeth (deep grooves and furrows); any crowding of teeth and the condition of old filling

Fluoride applications are done on children and adults past 3 years of age biannually, which leads to reduced cavity formation and also aids in stronger enamel formation. Its a mandatory procedure in mostly all developed countries.

Cavity in milk teeth are filled with tooth coloured fluoride releasing cements.

Fillings are done depending on area of tooth damaged and sometimes are followed by stainless still crown over them.

When the extent of cavity is deep almost approaching nerves these treatment modalities are opted followed by a stainless still crown over them.

When the cavity is very deep and already approaches nerve causing pain or swelling this procedure is carried out which is almost similar to root canal treatment in adults, followed by a stainless still crown over them.

Stainless Steel prefabricated crown are used to simulate the tooth structure lost due to cavity. Usually done after restorations, Direct/ Indirect pulp capping, pulpotomy or pulpectomy or clenching of teeth.

These are tooth coloured Zirconia crowns which come prefabricated in various sizes and are an alternative to stainless steel crowns for better aesthetics and also have good longevity.

Space Maintainers play a cardinal role when milk teeth are lost or extracted before the time of eruption of their permanent successor.

They maintain the space so that permanent teeth erupt in their specific places and further negates the chances of fixed orthodontic treatment in later ages.

Certain habits such as thumb sucking, lip bitting can cause adverse affect on growing jaws and teeth. Such habits can be corrected with the help of habit correctors.

Its a minor almost bloodless oral surgical procedure carried out with lasers in which frenum(tissue connecting teeth and lips/tongue is cut. It aids in better speech and proper tooth alignment (prevents gap between front teeth).

It is done with ultrasonic machine to remove any excess plaque and tartar deposit which can't be removed by normal brushing procedures.

Most children with special needs have greater dental treatment requirements. As paediatric dentists we receive advanced training that focuses on care for children with special needs. More than esthetics, functionality is the basic requirement for them to have nourishment. While some children may cooperate with behaviour modification methods, many would require treatment under general anaesthesia.

Most children with Special Needs may have a greater need for dental treatment for the following reasons:

•   Some children have increased susceptibility to tooth decay and gum disease.

•   Increased chances of falls and oral trauma.

•   Prescribed medication or diet detrimental to dental health(sweet syrups ).

•   Physical difficulty in maintaining oral hygiene.

However if dental care is started early and followed conscientiously, every child can enjoy a healthy smile. Dental disease in most cases is preventable, provided proper care being taken. No dentist can be better equipped than a pediatric dentist to treat a child with special needs. The advanced training that pediatric dentists undergo focuses on care for children with special needs. While some children may cooperate, many would require treatment under general anaesthesia.

Dentofacia offers cutting edge amenities for dental treatment under General Anesthesia. Complete dental treatment in one sitting under general anesthesia is the only effective treatment choice in many children who are extremely young (pre-cooperative, usually under the age of 3.5 years). Its a totally worthy treatment option for Children with Special Needs.

Conservative Dentistry And Endodontics

There are various types of tooth restorations depending on surface of the tooth involved, which are restored using tooth coloured fillings(cements/ composites) after removal of cavity.

Depending on the depth of the cavity and its distance from nerves inside, the filling can be indirect or direct pulp capping which is done using specialized cement.

Most common and intricate procedure carried out to save a deeply cavitated tooth where cavity has reached / infected the nerves causing pain. It can be single sitting or multiple sitting depending on the condition of tooth.

Tooth coloured restoration done on anterior(front) teeth using UV light. These are best esthetic alternative for small anterior tooth cavity/ fracture, also aids in smile designing.

This is a contemporary technique used to whiten the discoloured tooth/teeth and can be done in the clinic as well as at home by the patient depending on the criteria of discoloration.

A recent technique of teeth whitening using UV lights with reduced chances of sensitivity as compared to bleaching.

This is a specialized type of treatment done after root canal treatment for teeth with major loss of visible tooth structure, in which extra strength is provided by a metal or ceramic post(screw) followed by core(cement/composite) restoration.

Scaling and Polishing

Removal of soft and hard deposit from the tooth surface with help of ultrasonic instruments.

Removal of infected gum tissues causing bleeding gums by cleaning the surface of tooth covered by gums

Removal of severely infected gum tissues by giving incision ( cut ) and exposing the roots and underlying bone and followed by ocassional bone grafting for strengthening the mobile teeth and followed by suturing ( stitching ).

Procedure to enhance the available tooth structure for different treatment plans such as placement of cap and smile designing.

Procedure involving the lightening of hyperpigmented (dark) gum tissues to a lighter color of gums for better esthetics.

Metal braces

Traditional stainless steel brackets and stainless steel wire placed on the outer surface of teeth for correction of malaligned teeth.

Tooth coloured(ceramic) brackets used with stainless steel wire placed on the outer surface of teeth for correction of malaligned teeth. It is esthetically better as it reduces the metallic look of the smile.

The term “lingual” refers to the inside (tongue or palate) surface of the tooth. Lingual braces are attached to that inside surface unlike traditional braces which are fixed to the outside of the teeth. With the braces positioned on the inner surface, it makes them practically invisible and provides cosmetic orthodontic treatment without the distraction of typical braces.

Invisalign is a series of transparent aligners that are not fixed to teeth. Worn sequentially, the aligners progressively move teeth to their ideal position.

Successful outcomes require correct diagnosis and correct treatment implemented at the right age. It is a window of opportunity that is unique for every patient.

We identify problems early and nip things in the bud before problems snowball into the teenage years and become more complex to treat.

Crowns & Bridges

A crown is a type of dental restoration, which completely caps or encircles a tooth. Crown is indicated to restore a broken or damaged tooth or safeguard Root Canal treated teeth. They are typically bonded to the tooth using dental cement.

Types of crown –

•  Zirconia Crown (Bruzxir)

•  Ceramic Crown

•  Porcelain fused metal crown

•  Metal Crown

•  EMAX esthetic Crown

Dentures are a conventional, age old, tried and tested method of replacing missing teeth for the geriatric patients. Having complete dentures helps with chewing as well as supporting one's facial structure and improving ones speech. With immediate dentures, you can have a full set of teeth while you are waiting for your gums to heal and your bone to realign after having teeth removed. So, immediate dentures are a great alternative during that time.

Types of Dentures-

•  Complete Denture

•  Removable Denture(Flexible frame)

•  Cast Partial Denture (Metal Frame)

•  Fixed Partial Denture

•  Overdenture( denture supported by teeth/ implant)

Porcelain veneers commonly used as a treatment modality for Smile Enhancement are thin shells of ceramic that are bonded to the teeth in order to change the color, tooth position, form or shape and enhance the smile with an aesthetic appearance. They provide strength and resilience, which is comparable to natural tooth enamel.

Like any other form of architecture, designing the perfect smile is a unique combination of art and science. It requires a true creative spark on one hand, and in-depth knowledge, specialized skill and expertise on the other.

Oral Surgery

Wisdom Tooth Surgery

Wisdom tooth ( third molar ) comes the last is oral cavity and hence is placed in abnormal postion , needs to be extracted by giving incision (cut ) and removal of bone and suturing (stiching )

Porcedure done to remove apical lesion (tip of root inside bone)to save a tooth.

Extracting tooth for which tooth socket (bone boundary of tooth ) need not be violated.

Procedure to elevate the sinus lining( mucosa covering the air pocket in upper jaw) to place an implant

Replacement of missing tooth by placing titanium screw on with cap is placed later

Procedure to increase the bone width and length to place prosthesis (implant or gum preservation)

Procedure to improve width and condition of gingiva by placing soft tissue grafts

Removal of cyst ( abnormal cavity inside bone due to prolong irritation)

Reducing the size of cyst (abnormal cavity ) by removing the fluid inside the cyst

Using different type of bone materials to fill the space in jaw such as synthetic bone (tricalcium phosphate + hydoxyapatite) , autogenous bone( powder from patients jaw bone ) novabone ( xenograft), tata bone graft ( cadaveric bone graft )

Removing abnormal attachment of mucosa(skin) to the gums , or tongue to the floor of mouth

Using artificial Collagen protein( most common protein present is body for tissue regeneration 0 to regenerate bone tissue and soft tissue during healing .

Using blood of patient to make fibrin ( protein which accentuates healing ) by specialized centifugal machine .

Surgery done to refine , adjust the bone and mucosa ( skin inside the mouth ) for placement of prosthesis ( denture , implant )

Restriction of mouth opening by placing stainless steel wire around the teeth in cases of jaw fracture .

Surgical extraction of malposed canine tooth

Minimally invasive technique to extract tooth for application of braces for tooth alignment

Extraction of teeth which need incision ( cut) in the gingiva and bone cutting and exposure and suturing (stiching) post extraction

Jaw Surgery done under general anesthesia ( patient is unconcious , done in operation theatre ) for correction of abnormal position of upper or lower jaw.

Aesthetic Surgery

Hair transplant surgery

Hair transplantation means, transplanting live hair roots onto the bald areas (RECIPIENT AREA). These transplanted hair roots behave in the same manner as the hair from the area where they are extracted. This procedure, once done, revives patient’s confidence socially. It balances their emotional and psychological stress as well.

Surgery lifts and firms sagging facial tissues to restore a more natural youthful facial appearance. It helps a patient look like a younger version of him or herself. By removing excess, sagging skin, smoothing deep folds, and lifting and tightening the deep facial tissues, a facelift surgery can help a patient look not just younger, but simply "better".

Various options for face lift are as follows:

Standard facelift : Your surgeon will make incisions in your hairline at the temples, continuing down and around the front of your ears and hidden in the natural creases behind your ears in your lower hair-bearing scalp.

Mini facelift : Your surgeon will make short incisions in your hairline, starting at your temples and continuing down and around the front of your ears, hidden in the natural creases. There may also be incisions in the lower eyelids, temporal area, or under the upper lip.

Neck lift : Your surgeon will make incisions starting in front of your earlobes and continuing around behind your ears in your lower scalp. There will also be a small incision underneath your chin.

Cultural differences and individual preferences require changes in the aesthetics of the face. Lip reduction is a simple procedure done under local anesthesia which gives excellent results and totally changes the look of the face.

If both upper and lower lips are to be reduced than an ellipse of tissue is removed from the upper lip and two ellipses are removed from the lower lip. This is known as "Bikini Method" for lip reduction.

Nasal surgery (Rhinoplasty) is among the most common surgeries performed by Cosmetic Plastic Surgeons. Rhinoplasty is a facial cosmetic procedure, usually performed to enhance the appearance or reconstruct the nose. It can change the shape of the whole nose or a small part, making it fit more harmoniously with the rest of the face. If breathing problems are associated with the cosmetic deformity then they are also corrected simultaneously.

Surgery to improve the appearance of the eyelids. Many people develop the "tired eyes" look also known as baggy eyelids, saggy eyelids, droopy eyelids and/or puffy eyelids. It can be done on either on the upper lid or the lower lid or both.

Blepharoplasty may be an option if you have :

Baggy or droopy upper eyelids

Excess skin of the upper eyelids that interferes with your peripheral vision

Droopy lower eyelids, which may cause white to show below the colored part of the eye (iris)

Excess skin on the lower eyelids

Dimple Creation is a day care procedure which means you go home the same day after the surgery. It is done under local anesthesia by numbing the cheek skin where the dimple is to be created. The position of the dimple is as per the patients wishes or it can be determined with the help of the Plastic Surgeon.

Procedure :- It is performed from inside the mouth and does not leave any scar on the face. After the necessary surgical steps a suture is visible with a small dressing on the cheek at the sight of the dimple. This suture is removed after 7 days of the surgery.

Aesthetic Medicine

Botox treatment

Botox therapy is a very popular and easy way to look younger, and is perfect for that special occasion where the need to reduce fine lines and wrinkles is the need of the hour. Cosmetic Botox treatment is a prescription medicine that is injected into muscles and used to improve the look of moderate-to-severe frown lines between the brows in people 18 to 65 years of age for a short period of time.

Lightening and reducing redness, rosacea, dilated blood vessels and “broken” blood vessels on the face, neck and chest.

It also has a lesser effect of improving skin texture through collagen stimulation and improves pore size minimally by temporarily reducing oiliness options.

Dermal fillers are directly injected into the skin which plumps the area, removes wrinkle, fine lines and saggy skin. Dermal fillers are also used to lift jaw lines and cheeks, fill out thin lips and plump sagging hands. Dermal fillers are highly effective in the fight against signs of ageing.

Dermal filler for Nasolabial fold / Smile Line

The nasolabial fold is that the deep crease running from either aspect of the nose down towards the corners of the mouth. Conjointly called a "smile line," this crease is made by continual movement of the mouth. Whereas it's enticing to own to a small degree fold here, a deep fold will cause you to look older than you actually.

Dermal fillers Fillers-Lip

The lips measure the focal center of the lower face. It's forever necessary to think about antiviral bar in a very patient with a history of herpes simplex episodes.

The height of the lips and therefore the proportion of the higher to lower lip ought to be thought of. The lower lip ought to protrude slightly on the far side the higher lip. The higher lip ought to be 2/3 of the degree of the lower lip. Typically lip augmentation is finished with bigger particle-sized Dermal filler in distinction to lip line augmentation that is finished with smaller gel particle-sized filler.
